Department of Cellular Chimerism

The workplace is integrated into the Complement of laboratories IHBT. It continuously defends an independent and impartial assessment of the professional competence of the Complement of laboratories according to the standard ČSN EN ISO 15189, whose application is verified by the Czech Institute for Accreditation. The examination methods within the scope of accreditation are defined by the annex to the Accreditation Certificate in the current version, which is available on the website IHBT.

The basic focus of the department is the examination of cellular chimerism through genotyping and quantification of variable polymorphisms of the Short Tandem Repeats type (using PCR method and subsequent fragment analysis of PCR products on a genetic analyzer) and short insertions and deletions (using quantitative real-time PCR method). The examination is initially performed in patients indicated for allogeneic hematopoietic stem cell transplantation and their donors. Subsequently, long-term monitoring of the post-transplantation course takes place in patients, whereby chimerism analysis helps determine the dynamics of graft engraftment and monitors the possible return of the patient's hematopoiesis, which may indicate a risk of disease relapse. In the event of an increase in chimerism in patients after haploidentical transplantation, it is possible to determine whether there has been a loss of the HLA-mismatched haplotype in the patient's leukemic cells. The laboratory also analyzes peripheral blood samples from patients after organ transplantation, where it determines the proportion of organ donor hematopoiesis. 

The cellular chimerism department is a reference laboratory (National Reference Laboratory for DNA Diagnostics). It organizes external quality assurance comparisons and offers consultation services.

Trust and high data security is one of the basic priorities within the activities of the Complement of laboratories IHBT. The implemented procedures are carried out in accordance with the Personal Data Protection Act 110/2019 Coll., and the Regulation of the European Parliament and of the Council of the EU 2016/679, of 27 April 2016, on the protection of natural persons with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46/ES.
